생리통(월경통), Dysmenorrhea 2/3/2022

  • 사춘기 여아들의 대부분이 월경을 할 때 배가 아프지도 않고 아무 이상이 없는 것이 보통이다. 
  • 사춘기 여아들의 일부에게 월경이 시작되기 바로 몇 시간 전이나 하루 이틀 전 또는 월경을 하는 동안 월경으로 아랫배가 몹시 아플 수 있다. 월경통을 골반강 내에 이상이 없이 생기는 원발월경통(Primary Dysmenorrhea)과  자궁 내막증이나 자궁섬유증, 병적 문제가 있어 생기는 이차성 월경통(Secondary  Dysmenorrhea)으로 나눈다. 
  • 원발월경통이 있으면 재발성이 있고 하복부 통증이 있고 경련설 복토이 생길 수 있다.
  • 원발월경통이 있을 때 월경을 하는 것 이외, 배가 아픈 다른 이유는 없는 것이 보통이다. 
  • 전형적 생리통은, 월경을 시작하기 1~2일 전부터 시작할 수 있고, 월경이 시작되기 전 1~4시간부터 월경이 시작된 후 1~2일경에 더 심하고 그 후 1일 내지 며칠 간 계속될 수 있다. 
  • 사춘기 여아들의 생리통은 나이를 먹어가면서 좀 더 심해지는 것이 보통이다. 심한 생리통이 있을 때는 보통으로 하던 일과를 제대로 수행할 수 없다. 
  • 월경이 끝난 후에는 생리통은 더 이상 생기지 않는다. 
  • 월경으로 생기는 이런 통증을 생리통 또는 월경통이라 한다. 
  • 때로는 사춘기 여아들의 일부는 생리통으로 등교할 수 없다. 
  • 생리통과 다른 병과 감별 진단을 하기 위해 의사의 진단 치료를 받아야 할 때도 있다. 
  • 스테로이드와 진통제나 호르몬재 불임약으로 치료하기도 한다.

Dysmenorrhea

It is normal for most adolescent girls to have no stomach ache when they menstruate and there are no abnormalities. For some adolescent girls, menstruation can be very painful just hours before menstruation, a day or two before menstruation, or during menstruation.

There is usually no other reason for stomach pain other than menstruation.

Typical dysmenorrhea may begin 1 to 2 days before menstruation, from 1 to 4 hours before menstruation, more severe around 1-2 days after menstruation, and may continue for one to several days thereafter.

Menstrual cramps in adolescent girls usually get worse with age.

When you have severe menstrual cramps, you may not be able to perform your usual routine. After menstruation, menstrual cramps no longer occur.

Pain caused by menstruation is called menstrual cramps or dysmenorrhea. Sometimes, some adolescent girls can’t go to school because of menstrual cramps.

In order to differentiate menstrual cramps from other conditions, it is sometimes necessary to seek treatment from a doctor.
